Old Hammond Pediatrics is open 8:00 a.m. to 5:00 p.m.
Monday through Friday.
Office visits are made by appointments ONLY.

Medicaid and Billing:
Please call 1-800-259-4444 to pick Old Hammond Pediatrics
as your primary care facility. If you do not pick a doctor,
Medicaid/Community Care will assign a doctor to you.
OHP physicians, nurse practitioners, or registered nurses
cannot see patients who are linked to other doctors.
For patients whose primary care facility is not OHP,
you must have a transitional referral faxed to us in order
to be seen before your PCP change is effective.
